About the role

We are seeking a dedicated Registered Nurse (RN) to join our Women's Health Clinic team as a full-time Care Coordinator. The Care Coordinator will work with the team to support patients with chronic conditions and/or complex needs, developing, managing, and updating highly individualized care plans.

Responsibilities

• Support patients with chronic conditions and/or complex needs

• Work with the team to ensure comprehensive patient care

Qualifications

• Current Colorado RN license or ability to obtain BLS Certification through the American Heart Association
• Effective communication – both verbal and written
• Excellent interpersonal skills
• Able to affect change
• Able to perform critical analysis
• Self-directed
• Positive attitude
• Minimum 2 years RN work experience
• Three to four years RN experience in the area of specialty preferred
• Experience in Case Management a plus

$36.76 to $54.06/hour (BSN)
