Care Connections Nurse
Care Connections Nurse is part of a team designed to effectively assist patients in transitioning from hospital to community. The primary responsibilities include:
- Collaboration between hospital and community-based providers
- Arranging post-discharge services
- Working as a member of a multidisciplinary team
- Improving the patient’s health status
- Reducing the need for hospital care
- Assistance with program development, evaluation, professional education, or supervision of students
Job Requirements
- Applicable Experience: 1 year
- Basic Life Support - American Heart Association or Red Cross
- Registered Nurse - Various
- Bachelor's Degree - Science in Nursing
