RN Care at Home Field Case Manager in Buncombe County, NC

Primary Responsibilities

- Visit members in their homes to assess their current health status - Identify gaps or barriers in treatment plans - Provide patient education to assist with self-management - Make referrals to outside sources - Provide a complete continuum of quality care through close communication with members via in-person in home or on-phone interaction - Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels

Required Qualifications

- Current, unrestricted RN license in the state of North Carolina - Certified in Basic Life Support - 1+ years of clinical experience - Reside in or near Asheville, NC - Driver’s license and access to reliable transportation

Preferred Qualifications

- BSN - Certified Case Manager (CCM) - Case management experience - Experience in discharge planning - Experience in utilization review, concurrent review or risk management - Experience in a telephonic role - Experience in home care/home visits - Experience working with MS Word, Excel and Outlook - Background in managed care - Bilingual in English/Spanish

The salary for this role will range from $60,200 to $107,400 annually based on full-time employment. We comply with all minimum wage laws as applicable.
