RN CAP Case Manager - PRN

About the role

AuthoraCare Collective is seeking a PRN RN CAP Case Manager for our Greensboro service area. This position is hybrid. The role involves managing the overall plan of care for a minimum case load of CAP-C and/or CAP-DA recipients, assessing and identifying recipient/caregiver problems, establishing goals and interventions, and conducting monthly recipient visits to assess ongoing needs, interventions, and service utilization. Travel to and within patient homes in the service area is required.

Responsibilities

• Direct and coordinate all services provided through CAP-C and CAP-DA to recipients in accordance with the recipient’s individualized care plan.
• Utilize the case management process to plan and prioritize recipient care needs, including assessment, identification of problems, implementation of interventions, and evaluation of outcomes.
• Partner with the assigned RN case manager to complete the annual recertification process.
• Establish and maintain relationships with recipients/caregivers to assess ongoing needs.
• Recognize the influence of culture, ethics, and beliefs on decision-making.
• Conduct home visits in accordance with NC Medicaid requirements.
• Offer telephonic support to recipients and caregivers as needed.
• Communicate changes to the plan of care to recipients, families, and internal/external stakeholders.
• Comply with infection control and exposure control policies.
• Maintain effective relationships and partnerships with community healthcare providers and external stakeholders impacting the CAP-C and CAP-DA population.
• Complete required documentation according to licensure, certification, Medicaid requirements, and agency standards.
• Ensure notes and recipient records are complete, accurate, current, and clear.

Requirements

• Graduate of an accredited School of Nursing.
• Two years of experience in social work case management required.
• Additional two years of pediatric case management experience preferred.
• Case management accreditation or certification preferred.
• Valid state-issued driver’s license required.
• Must carry automobile liability insurance at limits required by agency.
• Must have own transportation.
