About the role
Provide case management services to clients in need. Assist clients in accessing necessary resources and support.
Responsibilities
- Assist clients in accessing healthcare, housing, and other essential services.
- Coordinate with community partners to ensure clients receive comprehensive support.
- Document client interactions and progress in CAP’s electronic health record system.
- Maintain confidentiality and adhere to all HIPAA guidelines.
Requirements
- Bachelor's degree in Social Work, Psychology, or related field.
- At least 2 years of relevant experience in social services or case management.
- Valid driver’s license and reliable transportation.
- Proficiency in Microsoft Office Suite.
Qualifications
- Strong interpersonal skills and ability to build rapport with clients.
- Excellent communication and organizational skills.
- Ability to work independently and as part of a team.
- Experience working with diverse populations.
Skills
- Knowledge of community resources and service providers.
- Basic computer skills including proficiency in Microsoft Office.
- Ability to manage multiple tasks and prioritize effectively.
