Purpose

The RN is responsible for monitoring the health and wellbeing of our residents and provides leadership support and assistance with resident care and services.

Responsibilities

  • Makes ongoing assessments of residents' health and wellbeing through the assessment / Personalized Service Plan (PSP) process
  • Ensures proper follow-through and documentation for residents with changing clinical care needs
  • Aids the Wellness Director in completing the clinical sections of monthly wellness visits and PSP to accurately reflect the resident's monthly needs
  • Informs the Wellness Director and/or Executive Director of any changes in medication or service levels that may necessitate a higher tier level or potential move-out
  • Aids with obtaining weights and vital signs monthly for each resident prior to completing monthly wellness health updates
  • Assists with direct resident care as needed

Education/Experience/License - Certification

  • Must be a Registered Nurse
  • A minimum of one year of experience working with elders or disabled individuals, preferred
  • Must have demonstrated Leadership capabilities
  • Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred

  • Understands advanced directives and end-of-life care
  • Understands and embraces the assisted living philosophy
  • Understands nursing function in assisted living, quality of care and quality of life objectives
  • Knowledgeable about state regulations and proficient in using state audit compliance tools
  • Proficient in time management skills and adherence to deadlines
  • Exceptional grammar and documentation skills
  • Understands basic concepts of risk management

Pay

  • $1500 sign-on bonus paid quarterly for up to 5-years of employment

Schedule

  • 7am-7pm; every other weekend required
  • 7pm-7am; every other weekend required
  • PRN shifts available
