About the role
The Residential RN (including traditional group homes (IRAs), ICFs, and Family Care residences) provides medical case management for individuals with developmental disabilities who are living in community-based homes. They supervise the completion of delegated nursing tasks to Direct Support Staff and LPNs/Nurse Associates as well as Family Care Providers (FCPs). They participate in interdisciplinary teams to provide nursing recommendations related to the individuals' overall plan of care, monitor and coordinate health care, and assist in coordinating medical care needs.
Responsibilities
- Participates as a member of the Interdisciplinary Team to provide nursing recommendations related to the individuals' overall plan of care.
- Makes nursing recommendations based on the individual's overall plan of care.
- Maintains compliance with all applicable State/Federal regulations and agency policy and procedure.
- Communicates issues and carries out all directives from Nursing Administration.
- Evaluates staff medication errors and ensures the plan of correction and retraining follows the Medication Error System.
- Provides direction to staff via phone or directs them to the contracted triage line per policy.
- Flexes schedule to provide services to individuals/sites as deemed necessary by the RN Supervisor.
- Participates in various nursing workgroups as assigned.
- Maintains communication with LPN and Nurse Associate (as applicable), senior residential supervisors, residential managers, nursing supervisor, family care management team, and other staff, as necessary.
- Attends weekly SharePoint/Teams meetings as required.
- Provides nursing coverage to all People Inc. programs as directed by Nursing Administration.
- Maintains CPR certification and performs CPR when necessary.
- Provides agency training related to nursing services as required.
- Complies with all agency policies and procedures.
- Other duties as assigned by Nursing Administration.
Requirements
- Current NYS license to practice as a Registered Nurse.
- NYS Driver’s License that meets agency policy.
- CPR certification.
- Physical agility and ability to react to emergency situations, including Strategies for Crisis Intervention and Prevention (SCIP) non-restrictive measures.
- Ability to meet physical requirements of the position when performing the following: standing, walking, sitting, bending, stooping, squatting, kneeling, and climbing.
Qualifications
- Experience in providing medical case management for individuals with developmental disabilities.
- Experience supervising nursing tasks to Direct Support Staff and LPNs/Nurse Associates as well as Family Care Providers (FCPs).
- Experience participating in interdisciplinary teams to provide nursing recommendations.
- Experience monitoring and coordinating health care delivery.
- Experience assisting in coordinating medical care needs using community medical providers and available clinical services.
- Experience providing health services training for staff and FCPs.
- Experience completing nursing responsibilities as indicated on the Nursing Responsibility Checklist for Residential Programs.
- Experience monitoring for completion of delegated nursing tasks to Direct Support Staff, Site Managers, LPNs, and Nurse Associates and FCPs.
- Experience monitoring medication administration in the facility.
- Experience evaluating staff medication errors and ensuring the plan of correction and retraining follows the Medication Error System.
- Experience maintaining compliance with all applicable State/Federal regulations and agency policy and procedure.
- Experience communicating issues and carrying out all directives from Nursing Administration.
- Experience presenting to the assigned site when notified that state or federal survey has arrived.
- Experience participating in plan of correction related to medical citations resulting from any internal or external survey.
- Experience maintaining communication with LPN and Nurse Associate (as applicable), senior residential supervisors, residential managers, nursing supervisor, family care management team, and other staff, as necessary.
- Experience attending weekly SharePoint/Teams meetings as required.
- Experience providing nursing coverage to all People Inc. programs as directed by Nursing Administration.
- Experience maintaining CPR certification and performing CPR when necessary.
- Experience providing agency training related to nursing services as required.
- Experience complying with all agency policies and procedures.

Pay
$76,960 - $83,200
Schedule
This is not a remote position. Any work that is remote will be at the discretion of the AVP of Nursing or the VP of Nursing. Will participate in cross training within the nursing areas if nursing coverage is needed in areas other than residential sites.
