Responsibilities
- Conduct thorough assessments of residents' medical and care needs
- Develop and implement care plans based on assessments
- Maintain accurate documentation for reimbursement purposes
- Coordinate care delivery and monitor resident progress
- Ensure compliance with regulatory requirements
- Support, advise, coach, and influence team members, residents, and family members
Requirements
- Possess a valid RN license issued by the State of Pennsylvania and maintain current CPR certification
- Minimum 1 year of long-term care experience; supervisory experience in a skilled nursing environment is preferred
- A minimum of 2 years of experience with completing MDS and facilitating interdisciplinary care planning
- Strong interpersonal skills to support, advise, coach, and influence others
- Proficiency with MDS software as well as PC and Microsoft Office applications
