RN, Ambulatory Clinic - Infectious Disease

About the role

The Staff RN-Ambulatory Clinics works collaboratively as an active member of the interprofessional team to provide therapeutic patient and family centered care. The Staff RN-Ambulatory Clinics systematically and continuously collects and assesses data related to patient health status to develop, execute and evaluate the plan of care.

The Staff RN-Ambulatory Clinics utilizes evidence to drive clinical decisions and practice related to individual and population health. The Staff RN-Ambulatory Clinics establishes goals and strategies for meeting care coordination needs across settings with the patient, family, and/or other care providers.

This role is located at our CP-TOWN HALL HEALTH CENTER facility in Menomonee Falls, WI, and is an on-site position with a schedule of Monday-Thursday, 8:00 AM - 4:30 PM.

Responsibilities

Major accountabilities of this role include patient care coordination, direct patient care, patient education, telephone triage/eVisits, and performing/assisting with in-clinic minor procedures. May float to other areas on as needed for operations need. Other duties as assigned.

Requirements

• Clinical experience and/or interest in the care and management of patient populations specific to the area of employment
• At least 1 year of relevant nursing experience is preferred
• Nursing Licensure Required
• Bachelor's Degree In Nursing, Preferred
• Requires current state of Wisconsin Registered Nurse License or Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact)
• American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within 90 days of hire
