Responsibilities
- Ensures patient flow in a 24/7 environment.
- Maintains critical access for specialty services.
- Balances regional needs while facilitating timely admission of patients to Strong Memorial Hospital.
- Facilitates timely transfer of patients from acute care hospitals to either Strong Memorial Hospital or Affiliate Hospitals based on patient needs.
- Acts as a liaison between referring physicians and accepting physicians to facilitate communication regarding the smooth transfer of acute care patients.
- Trips appropriately to attending physician and serves as required.
- Uses clinical judgment to ascertain level of care discrepancies and seeks direction from internal physicians when necessary.
- Acts as a clinical resource person and collaborates with patient care areas and other departments to ensure the needs of patients are safely met and institutional needs are met.
- Assumes a leadership role in collaborative practice, patient flow improvements, and other regional care activities.
- Supports flow initiatives and supports Nursing, Specialty Service, and General Services in understanding census limitations.
- Makes critical decisions in a timely manner and exhibits knowledge of, and ensures, compliance with Hospital Policy, Federal and State regulatory requirements.
- Reviews and evaluates regional needs for patient transfer to and between Affiliate Hospitals.
- Leverages clinical knowledge and business acumen to make appropriate decisions when transferring patients within SMH, Intra Affiliate, and Regional Institutions.
Requirements
- Minimum Education & Experience: Bachelor's degree in Nursing or Health Related Field, 3 years' experience preferred.
- Licenses and Certifications: RN - Registered Nurse - State Licensure and/or Compact State Licensure upon hire required.
Qualifications
- Utilizes clinical skills to manage patient flow in an expert manner, following policies and procedures.
- Recognizes trends or subtle changes in patient /family response to care and responds effectively.
- Identifies complex patient problems and collaborates in developing a plan of care based on best practice standards and service availability.
- Determines best patient placement based on expert clinical knowledge, current occupancy, and staffing.
- Identifies patient safety issues and implements action plans or educational training based on needs.
- Demonstrates exemplary unit/service standards of documentation.
- Performs ongoing comprehensive assessment of regional, institution, department, and service components that contribute to patient flow.
- Provides information to update practice standards and activities based on best evidence.
- Evaluates the effectiveness of patient placement based on clinical needs and revises as indicated.
- Functions effectively with clinical staff as an expert Patient Flow Manager.
- Collaborates with patient care areas and other departments to ensure the needs of patients are safely met and institutional needs are met.
