About the role
This role involves evaluating and admitting patients into hospice care, conducting comprehensive clinical assessments, coordinating with physicians and the interdisciplinary team, and completing all required documentation accurately and timely.
Responsibilities
- Conduct comprehensive clinical assessments
- Congratulate physicians and the interdisciplinary team
- Complete all required documentation accurately and timely
- Serve as a key point of contact for patients, families, and referral sources
- Provide education on hospice philosophy, goals of care, and end-of-life support
- Facilitate a smooth transition into hospice by initiating care plans and ensuring all services are in place in different care settings
- Assess patients for hospice eligibility
- Coordinate admissions
- Ensure timely, accurate documentation
- Educate patients and families on hospice care
- Collaborate with the care team
- Help ensure a smooth transition into services
Qualifications
- Bachelor’s degree in Nursing or related field
- Current licensure to practice as a Registered Nurse (RN)
- Experience in hospice or palliative care preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
