RN - ACT - Assertive Community Treatment

About the role

Join our dynamic ACT (Assertive Community Treatment) Team as a Registered Nurse. In coordination with other ACT team members, you will provide, oversee, and/or administer a wide range of psychosocial evaluation/assessment, diagnostic, counseling therapy, and crisis intervention services in a clinical environment. This role requires a high degree of independent decision-making as well as coordinated interdisciplinary teamwork. Most aspects of this job involve clinical assessment, evaluation, and intervention skills for persons with severe and persistent mental illness who need intensive services within a community-based setting.

Area of travel: Anson, Richmond, Montgomery, and Randolph Counties. A company vehicle will be provided.

Responsibilities

• Provides medication evaluation, medication checks, calls in prescriptions, obtains samples, and may provide individual or family therapy to persons served.
• Participates in daily interdisciplinary treatment planning, daily staff meetings, and review of persons served and their medical needs.
• Provides crisis intervention consultation along with all members of the team; may provide on-call coverage on a rotating or PRN basis.
• Provides ongoing assertive outreach, therapeutic interventions, and supportive services to address the needs of persons served effectively.
• Provides interventions to offer support and guidance in all areas of functional domains: adaptive, communication, personal care, domestic, psychosocial, problem-solving, etc., to prevent, overcome, or manage the person's level of functioning and enhance their ability to remain in the community.
• Completes all documentation as required by procedures and statute.
• Participates in regular supervision with the ACT Team Leader and/or psychiatrist to enhance all competencies in this position.
• Along with other team members, ensures adequate service delivery is provided to all persons served in accordance with National Program Standards, state service definition requirements, and the needs of the persons served.

Requirements

• Strong psychosocial and clinical assessment skills.
• Ability to work with minimal supervision regarding use of time and prioritize work assignments.
• Ability to communicate effectively with professionals and clients/families.
• Ability to make sound decisions in emergency situations.

Qualifications

Graduation from a four-year state-accredited school of nursing and one year of experience in psychiatric nursing; or an equivalent combination of education and experience.

Experience and skills in vocational/rehabilitation treatment/counseling are required. Case management and supportive services are also integral to this position.

Pay

$50 an hour, with access to a company vehicle.
