Responsibilities
- Perform comprehensive patient assessments, including medical histories, physical examinations, and diagnostic evaluations.
- Evaluate and manage patients with rheumatologic conditions such as: Rheumatoid Arthritis, Psoriatic Arthritis, Ankylosing Spondylitis, Systemic Lupus Erythematosus, Vasculitis, Gout, Osteoporosis.
- Order, interpret, and communicate laboratory tests, imaging studies, and other diagnostic procedures.
- Develop and implement individualized treatment plans under established clinical protocols.
- Prescribe medications and therapies in accordance with state regulations and organizational policies.
- Maintain treatment efficacy, adverse effects, and disease progression through ongoing monitoring.
- Initiate and manage conventional, biologic, and targeted therapies.
- Monitor patients receiving immunosuppressive medications and biologic agents.
- Collaborate with infusion centers, specialty pharmacies, and insurance providers regarding medication authorizations and treatment plans.
- Ensure appropriate laboratory monitoring and medication safety practices.
- Provide education to patients and families regarding disease processes, treatment options, medication administration, and lifestyle modifications.
- Promote adherence to treatment plans and preventive health measures.
- Offer counseling regarding symptom management and self-care strategies.
- Carefully coordinate with physicians, nurses, pharmacists, physical therapists, and other healthcare professionals.
- Collaborate with specialists as needed and participate in multidisciplinary case reviews and quality improvement initiatives.
- Aid in transitions of care and follow-up management.
- Maintain accurate, timely, and complete electronic medical records.
- Ensure compliance with organizational policies, clinical guidelines, and regulatory requirements.
- Participate in quality assurance, patient safety, and performance improvement activities.
Qualifications
- Graduate of an accredited Nurse Practitioner or Physician Assistant program.
- Current state license as an Nurse Practitioner or Physician Assistant.
- BLS upon hire and maintain current.
- Two years of experience as an APP preferred.
- Previous experience with electronic health record.
