Rheumatology Advanced Practice Provider

Responsibilities

• Perform comprehensive patient assessments, including medical histories, physical examinations, and diagnostic evaluations.
• Evaluate and manage patients with rheumatologic conditions such as: Rheumatoid Arthritis, Psoriatic Arthritis, Ankylosing Spondylitis, Systemic Lupus Erythematosus, Vasculitis, Gout, Osteoporosis.
• Order, interpret, and communicate laboratory tests, imaging studies, and other diagnostic procedures.
• Develop and implement individualized treatment plans under established clinical protocols.
• Prescribe medications and therapies in accordance with state regulations and organizational policies.
• Maintain treatment efficacy, adverse effects, and disease progression through ongoing monitoring.
• Initiate and manage conventional, biologic, and targeted therapies.
• Monitor patients receiving immunosuppressive medications and biologic agents.
• Collaborate with infusion centers, specialty pharmacies, and insurance providers regarding medication authorizations and treatment plans.
• Ensure appropriate laboratory monitoring and medication safety practices.
• Provide education to patients and families regarding disease processes, treatment options, medication administration, and lifestyle modifications.
• Promote adherence to treatment plans and preventive health measures.
• Offer counseling regarding symptom management and self-care strategies.
• Carefully coordinate with physicians, nurses, pharmacists, physical therapists, and other healthcare professionals.
• Collaborate with specialists as needed and participate in multidisciplinary case reviews and quality improvement initiatives.
• Aid in transitions of care and follow-up management.
• Maintain accurate, timely, and complete electronic medical records.
• Ensure compliance with organizational policies, clinical guidelines, and regulatory requirements.
• Participate in quality assurance, patient safety, and performance improvement activities.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current state license as an Nurse Practitioner or Physician Assistant.
• BLS upon hire and maintain current.
• Two years of experience as an APP preferred.
• Previous experience with electronic health record.
