Responsibilities
- Provide intermittent skilled nursing services in accordance with the established plan of care under RN direction.
- Support patient/family using teaching principles to promote and restore health, and prevent disease and disability.
- Record daily progress in the Electronic Medical Record (EMR).
- Report to primary nurse, supervisor, physician, social worker, or therapist regarding cases.
- Communicate and conference with primary nurse and other disciplines.
- Consult with physicians regarding client needs and plan of care.
- Assist in evaluating care provided by paraprofessional staff.
- Participate in agency committees and special projects.
Requirements
- High School diploma or GED.
- Successful completion of a CT state-approved LPN certificate program.
- Current and valid LPN license.
- Valid CT driver’s license and a good driving history.
Qualifications
- Experience in a healthcare or homecare setting.
- Computer skills including Outlook, Microsoft Word, and Excel.
