What You Bring
Education: High School completion required; Associate’s degree in a Social Services field preferred.
Experience:
- Associate’s degree requires two (2) years of experience performing non-professional, consumer-directed services in behavioral health, healthcare, or social services.
- High School completion requires an additional two (2) years of experience in a social service field.
Licensure & Compliance: A valid California Driver’s License and eligibility to meet agency driving criteria.
Responsibilities
This role assesses clients' housing, education, employment, and resource needs, develops individualized intervention plans, and connects clients with appropriate community services. The position collaborates with treatment teams, maintains community partnerships and resource networks, coordinates transportation and support services, participates in outreach efforts, and ensures accurate and timely documentation in the Electronic Health Record (EHR) system.
Division/Program Overview
- Population Served: Serving adults, 18 plus.
- Facility & Setting: Outpatient community-based services and support. Delivers client services throughout Fresno County within designated county limits. May be required to transport clients/client family members utilizing personal vehicle.
- Clinical Services: Individual Counseling.
- Skills & Services: Medication management, Housing, Education, and Vocational Support services.
- Role-specific Benefits & Reimbursements: Mileage reimbursement included at a rate of $0.85 per mile and Monthly stipend included (Cell phone / Internet).

Working Location: Community-based in Fresno County
Working Hours/Shift: M-F 8AM-5PM. Working hours are approximate and may include flexibility as needed to meet client needs.
Starting Compensation: 22.07 - 28.00 USD Per Hour
