About the Role

Ensures the delivery of quality medical services to individuals with Intellectual Disability / Developmental Disability living within the CLS IRA program. Acts as the primary liaison between medical service providers and individuals, and provides supervision of unlicensed direct care staff in the performance of nursing tasks and activities.

Responsibilities

  • Participates in the medical on-call system.
  • Ensures medical care for individuals within a designated cluster, including developing and updating individualized nursing service plans annually.
  • Tracks and ensures completion of all medical services, testing, and consults as recommended by providers.
  • Ensures prompt rescheduling of any cancelled medical appointments.
  • Accompanies individuals on critical medical appointments to enhance communication and coordination of services.
  • Conducts weekly visits to residences for medical monitoring and review of medical reports, diagnostic tests, and laboratory results.
  • Completes monthly site visits, including medication oversight and reconciliation.
  • Completes nursing progress notes monthly and during illnesses until resolved.
  • Tracks medical appointments using agency forms and reviews them with AD Nursing during monthly support meetings.
  • Attends monthly support meetings with AD Nursing to review health status and discuss job-related concerns.
  • Provides initial and ongoing supervision, training, and support to unlicensed direct care staff on individuals’ chronic medical conditions and nursing tasks.
  • Conducts annual clinical performance evaluations for unlicensed direct care staff, including medication administration (AMAP pouring).
  • Monitors and visits hospitalized individuals at least weekly and coordinates communication between the hospital and the Habilitation Team.
  • Acts as the Agency’s lead contact for all discharge planning.
  • Participates in a 24/7 on-call system to provide direction and support to Residential Staff, including verification of new medication orders.
  • Ensures back-up nursing coverage during accrued agency time or coworker leave.
  • Provides a daily nursing report to the supervisor regarding any changes in individuals’ health status.
  • Ensures pharmacy reviews are completed every 6 months for IRAs and quarterly for ICFs, and follows up on recommendations.
  • Leads team meetings for individuals prior to scheduled surgeries, procedures requiring anesthesia, or other significant medical concerns.
  • Ensures Compass and State audits are prioritized and attends them as necessary.
  • Contacts family members to secure consent for medical procedures, pre-sedation, pre-medication, and psychotropic medication initiation.
  • Coordinates and presents medical information for Surrogate Decision Making Committee review.
  • Notifies supervisor immediately if unable to perform any job responsibility on a weekly basis.
  • Requests time off as per agency handbook and notifies supervisor of schedule changes or sickness.
  • Drives between sites and for in-person meetings as necessary to perform job responsibilities.
  • Ensures work-related safety procedures are followed and reports unsafe acts or hazardous environments.
  • Reports to the Justice Center as a mandated reporter if there is reasonable cause to suspect abuse or neglect.
  • Adheres to all regulations, agency policies, procedures, and training requirements.
  • Arrives to work on time and adheres to agency policy regarding call-offs and overtime.
  • Performs other duties as requested by supervisor.

Requirements

  • NY State licensed Registered Nurse.
  • Current CPR certification.
  • One year of experience working with the ID/DD population is preferred.
  • Ability to drive is necessary to perform job responsibilities.
  • Excellent oral/written communication and organizational skills.
  • Subject to successful completion of fingerprint/criminal background check due to regular and substantial unsupervised contact with individuals receiving services.
