About the role
Bancroft is seeking a Residential Nurse Case Manager to join our Cherry Hill, NJ team. This position is part of a nonprofit organization dedicated to supporting people with special needs and their families.
Responsibilities
- Promotes the health and wellness of individuals supported
- Serves as key liaison, advocate, and resource of information regarding healthcare needs
- Facilitates a team approach to providing comprehensive care
- Communicates effectively with family members, healthcare providers, and other stakeholders
- Identifies, assesses, and improves the quality of life and healthcare for individuals served
Requirements
- Registered Nurse with a current and valid N.J. license
- Minimum of two years of general nursing experience, with three years preferred
- Education: Degree in Nursing or certification in Case Management/Developmental Disability, Brain Injury Specialist, Certified Rehabilitation Registered Nurse (CRRN)
Qualifications
- Experience working with individuals with developmental disabilities, brain injuries, and neurological impairments
- Knowledge of relevant regulations and guidelines (Nurse Practices Act, DDD, NJDOH, NJDOE, CARF, JCAHO, DCF)
- Ability to communicate effectively with a diverse range of stakeholders
Skills
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Proficiency in using electronic health records and other relevant software
