About the program:

Willow House is a 5-bed, co-ed 24-hour staffed intensive mental health program for young adults, ages 18-25, located in Enfield. This is a group home setting. Staff work with clients individually and in groups to teach independent living skills, such as cooking, cleaning, budgeting, hygiene, and how to access resources in their community. Staff also support educational and/or vocational goals and teach interventions to manage their mental health needs. The goal of Willow House is to provide support and skills necessary for our young adults to be able to live independently in the community.

Duties & Responsibilities

  • Maintain physical environment and program standards to CHR, DMHAS, JCAHO and any other licensing standards
  • Monitor medications of all residents in accordance with the Adult residential medication policy and procedure
  • Effectively utilize appropriate communication methods/tools to transfer information to other staff regarding the day's events
  • Teach and assist all residents in areas of rehab including: nutrition and healthy living, meal preparation, hygiene, laundry, banking and budgeting, and basic home care skills
  • Absorb administrative functions and tasks at the direction of the supervisor
  • Prepare and facilitate educational and skill-building groups and one-to-one interventions as directed by the treatment plan
  • Collaborate and coordinate care with providers and families as needed
  • Assist residents in addressing medical and psychiatric needs
  • Maintain documentation in compliance with agency and program standards
  • Participate in staff meetings, supervision, trainings and other meetings as assigned
  • Additional duties as required

Qualifications

  • High diploma or GED required
  • Two years’ experience working with a relevant population preferred
  • Valid driver’s license required with good driving record
