About the role

The Residential Counselor reports to the group homes' Residential Coordinator and works under the clinical supervision of the Rehabilitation Director. They are responsible for providing direct services that promote participation in community activities and the development of independent living skills. The role involves assessing clients' rehabilitative needs, developing and implementing individual rehabilitation plans, and coordinating activities that enhance community integration.

Responsibilities

• Participates in the assessment of clients' rehabilitative service needs and develops, implements, and monitors individual rehabilitation plans.
• Delivers rehabilitation and support services aimed at enhancing independent living skills, including personal hygiene, grooming, menu planning, socialization, and money management.
• Provides basic supervision in home-like environments for individuals with varying levels of mental illness.
• Coordinates activities that promote community integration and communicates with program staff about client progress.
• Documents service delivery and prepares daily billing statements as directed.
• Makes rounds to ensure assigned vehicles are maintained appropriately.
• Collects and records data as required, monitors resident and program documentation, and participates in discharge planning and follow-up services.
• Attends staff meetings, training sessions, and program meetings as needed.
• Maintains client and program compliance with VOAC policies and procedures.
• Maintains homes inspection-ready and reports any issues to the Residential Coordinator.
• Performs other duties as assigned.

Qualifications

• High School Diploma or GED with at least two years of experience working in a mental health residential setting or social service-related field.
• Preferred: Bachelor's Degree in related field or Registered Nurse licensure with at least 1 year experience providing rehabilitative services or working with persons with chronic mental illness.
• Excellent written and verbal communication skills.
• Experience in a fast-paced healthcare environment.
• Experience with Microsoft Suite (Powerpoint, Excel, Word).
• Bilingual in any language is a plus.
