About the role

Lifeworks provides direct care to individuals through supervision, role modeling, and teaching. The position adheres to Human Rights, D.P.P.C., Department of Developmental Services, and Division of Medical Assistance guidelines.

Responsibilities

• Promotes optimal health through routine, preventive, and emergency health care.
• Safeguards residential environments, ensuring safety and freedom from harm.
• Trains, counsels, and assists individuals in achieving ISP goals across various domains.
• Develops and presents annual ISP's and semiannual assessments in collaboration with other staff and the Director.
• Provides transportation as necessary and communicates with significant others to maintain consistency in programming.
• Maintains individual confidential files, reviews cases, documents activities, and trains.
• Demonstrates good computer skills and learns to utilize agency’s computer programs for necessary data documentation.
• Attends regular meetings as needed and performs supervisory duties as required.
• Works at various locations based on the company’s needs.

Qualifications

• BA/BS in Human Services or related field, or associate degree with 3 years of relevant experience with persons with ID/DD.
• Associate degree with 5 years of relevant experience in Human Services or a high school diploma/GED with 5 years of relevant experience with persons with ID/DD or human services.
• Ability to read, write, and communicate effectively.
• Computational skills, including basic math and data management.
• Problem-solving and interpersonal skills.
• Valid and current driver’s license, Medication Administration, First Aid, and CPR certifications.

Pay

$47,133 per year
