About the role
Lifeworks is a nonprofit human service agency that provides adults with developmental disabilities with the opportunity to live engaged and meaningful lives. We are seeking a Teacher/Counselor (Residential Counselor) to work with individuals in one of our Residential Programs.
Responsibilities
- Promotes optimal health for individuals through routine, preventive, and emergency health care.
- Assists with all hands-on direct care which includes but not limited to: showering, toileting, shaving, brushing teeth, meal-time assistance.
- Ensures that residential environments are safe and secure and where individuals are free from harm.
- Provides education training, counseling, and physical assistance when necessary to achieve ISP goals in the following areas: functional academics, domestic skills, health and personal hygiene, social and leisure skills, communication skills, community mobility, self-esteem, and money management.
- Develops, writes, and presents annual ISPs and semi-annual assessments in consultation with other residential staff and directors.
- Communicates with persons significant to the individual served (i.e., family, friends, guardians, funding sources, advocates, volunteers, employers) to facilitate consistency of programming and support.
- Performs case management duties including, maintenance of individual confidential files, ensuring cases are reviewed, documentation of activities, and training.
- Attends regular meetings as necessary.
Requirements
- Experience working with Individuals with ID/DD
- A Bachelor's degree or an associate degree in a related field with two years of experience, or an HS diploma and five years of related work experience in Human Services.
- Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks.
- Valid Driver's License required.
- Valid MAP certification is preferred.

Schedule
Sunday 9am-2pm, Thursday 3pm-Friday 9am, and Saturday 8am-Monday 9am
