3rd Shift, Sunday–Thursday, 11 p.m.–7 a.m. (40 hours/week with $1.00/hour weekend shift differential; base rate $21.17/hour).

About the Role

Assists individuals with daily personal routines at the New Haven Halfway House. Provides prompts for daily living skills, skill-building activities, and participation in groups. Offers companionship, establishes trusting professional relationships, and collaborates with nursing and management to ensure high-quality resident services.

Responsibilities

  • Establish rapport and positive working relationships with clients while maintaining professional ethics and boundaries.
  • Enhance clients’ independent living skills through modeling, prompting, and hands-on assistance.
  • Provide hands-on support for activities of daily living: personal care/hygiene, housekeeping, laundry, food management/nutrition, budgeting, health and safety, and leisure.
  • Facilitate or co-facilitate psychoeducational groups.
  • Engage clients in community integration and social recreation activities.
  • Provide transportation to and from appointments; educate clients on travel planning and use of public/private transportation.
  • Conduct regular apartment/room inspections and assist clients in maintaining clean living spaces.
  • Perform ongoing safety rounds and administer breathalyzer tests and urinalysis samples as applicable.
  • Document shift logs in electronic records according to contract and agency policy.
  • Develop and maintain relationships with providers, families, and natural supports.
  • Respond to crisis situations using Nonviolent Practices as outlined in agency policy.
  • Supervise, monitor, and document self-administration of client medications.
  • Attend monthly staff meetings, clinical rounds, All-Treater meetings, and supervision.
  • Maintain a clean, well-organized work environment; ensure program egresses and walkways are clear for safety.
  • Notify supervisor of maintenance/safety issues and generate work orders as needed.
  • Participate in fire/emergency evacuation drills and review emergency preparedness plans.
  • Adhere to policies in the staff handbook and program operations manual; uphold HIPAA regulations.
  • Model culturally competent behavior with clients, providers, families, and staff.
  • Maintain accurate electronic records and case files per programming, contractual, licensing, and accreditation requirements.
  • Adhere to CARF (Council on Accreditation of Rehabilitation Facilities) standards.
  • Complete required trainings and attend agency-sponsored in-services, outside trainings, and conferences.
  • Remain awake during all shifts to maintain security and safety of clients and facility.

Requirements

  • High School Diploma or equivalent with a minimum of one (1) year experience in a related healthcare field.
  • Licensed CNA or other related healthcare certification accepted in lieu of experience.
  • Prior experience working with people with severe and persistent mental illness, intellectual disabilities, and/or addiction.
  • Knowledge of community support systems and resources preferred.
  • Valid Connecticut Driver’s license with a clean driving record.
  • Reliable transportation required; willingness to transport clients in personal vehicles.
  • CPR/First Aid certification (offered during orientation; employee responsible for maintaining ongoing certification).
  • Proficient computer skills, including Microsoft Office and electronic record keeping.
