Resident Services Coordinator - Independent Living, Full-Time

About the role

This role involves informing residents and families about available health and support services, developing contacts with service providers for referrals, educating residents on services and application processes, and monitoring service provisions to ensure quality care. The coordinator conducts resident reviews to assess eligibility, provides limited case management, and supports residents' community engagement and well-being.

Responsibilities

• Conduct resident assessments for admittance and continued residence
• Monitor and ensure appropriate, timely delivery of services
• Educate residents on available services, rights, and application procedures
• Refer residents to community volunteer programs and support organizations
• Assist management in identifying residents needing additional help
• Maintain resident files and documentation
• Support residents' active community participation and overall well-being
• Provide limited case management when needed

Qualifications

• Experience in social work, healthcare, mental health, or gerontology, with at least 2 years working with older adults
• Effective crisis response and excellent customer service skills
• Ability to read, write, and communicate fluently in English

Physical & Working Conditions

• Light physical effort, handling objects up to 20 pounds occasionally
• Frequent sitting, standing, bending, and interaction with residents and staff
• Ability to manage interruptions and multitask
