Resident Services Coordinator - Independent Living, Full-Time

Position Summary This role involves informing residents and families about available health and support services, developing contacts with service providers for referrals, educating residents on services and application processes, and monitoring service provisions to ensure quality care. The coordinator conducts resident reviews to assess eligibility, provides limited case management, and supports residents' community engagement and well-being. Key Responsibilities Conduct resident assessments for admittance and continued residenceMonitor and ensure appropriate, timely delivery of servicesEducate residents on available services, rights, and application proceduresRefer residents to community volunteer programs and support organizationsAssist management in identifying residents needing additional helpMaintain resident files and documentationSupport residents' active community participation and overall well-beingProvide limited case management when needed Qualifications & Skills Experience in social work, healthcare, mental health, or gerontology, with at least 2 years working with older adultsEffective crisis response and excellent customer service skillsAbility to read, write, and communicate fluently in English Physical & Working Conditions Light physical effort, handling objects up to 20 pounds occasionallyFrequent sitting, standing, bending, and interaction with residents and staffAbility to manage interruptions and multitask
