About the role
The Resident Service Coordinator will join our team at Greeley Manor, a 128-apartment-home community for seniors. This role focuses on improving residents' quality of life by connecting them with resources, programming, and support services.
Responsibilities
- Develop relationships and partnerships with local agencies and nonprofits to provide services for residents.
- Link and develop ongoing educational programming, wellness presentations, and on-site events that empower residents and promote self-sufficiency.
- Demonstrate working knowledge of federal, state, and local resources, including entitlement and means-tested programs for seniors.
- Serve as a liaison with medical, professional, and community service providers upon resident request or authorization.
- Report abuse, neglect, or exploitation of residents in compliance with state statutes and ethical standards.
- Provide limited case management when requested and authorized by the resident (no direct services).
- Maintain resident files in compliance with HUD and Silver Tree Residential requirements, including preparation of regulatory reports.
- Develop and maintain a resource directory for the apartment community.
- Assist residents, families, and representatives with transitions into and out of the facility (consumer-directed).
- Accompany the Community Director on apartment inspections and coordinate plans to help residents age in place.
- Follow up on referrals from property management inspections.
- Create a monthly calendar and site newsletter with input from the Community Director and staff.
- Attend home visits of prospective tenants to discuss the Service Coordination program and assist with move-in.
Requirements
- Bachelor’s degree in gerontology, social work, or a related field (Master’s degree or AASC Professional Services Coordinator certification preferred).
- Minimum of three years of experience providing social services, with a focus on the elderly.
- Knowledge of legal liability issues related to providing social services to at-risk populations.
- Strong commitment and passion for serving an elderly population with physical, functional, social, and cognitive challenges.
- Excellent written and verbal communication skills, with the ability to problem-solve, educate, and advocate for vulnerable adults.
- Strong organizational skills and proficiency in MS Office, including Excel.
