About the role

Silver Tree Residential, LLC is seeking a dependable and experienced Service Coordinator to join their team at Cumberland View Towers, a high-rise community for seniors.

Responsibilities

  • Develop relationships and partnerships with multiple agencies and nonprofits to provide services for residents.
  • Link and develop ongoing educational programming, wellness presentations, and on-site events to promote resident self-sufficiency.
  • Demonstrate a solid working knowledge of federal, state, and local resources, including entitlement and means-tested programs for seniors.
  • Serve as a liaison with medical and professional services providers, and community agencies upon request or authorization from the resident or their legal representative.
  • Report abuse, neglect, or exploitation of a resident in accordance with state statutes and ethical practice guidelines.
  • May provide limited case management when requested and authorized by the resident.
  • Maintain resident files in compliance with HUD and Silver Tree Residential requirements, preparing and submitting HUD regulatory reports as directed.
  • Develop and maintain a resource directory for the apartment community.
  • Accompany the Community Director on apartment inspections and collaborate with residents to continue aging in place.
  • Follow up on referrals from the property management team.
  • Create a monthly calendar and site newsletter with input from the Community Director and other staff.
  • Attend home visits of prospective tenants to engage in early communication about the Service Coordination program and assist in the move-in process.

Qualifications

  • Minimum of a bachelor’s degree in gerontology, social work, or a related field.
  • Master’s degree and/or AASC Professional Services Coordinator certification preferred but not required.
  • At least three years of experience providing social services.
  • Genuine interest in continuing education that directly serves the elderly.
  • Informed about legal liability issues relevant to the provision of social services to at-risk populations.
  • Sincere commitment and passion to serve an elderly population characterized by physical, functional, social, and cognitive challenges.
  • Strong written and verbal communication skills, and the ability to problem-solve, educate, and advocate for vulnerable adults.
  • Excellent organizational skills and experience with MS Office and Excel spreadsheets.
