About the role

The Service Coordinator I position at National Church Residences- All Divisions is responsible for coordinating programs and services to help participants maintain a good quality of life and age in place. Under the supervision of the Property Manager/Staffing Manager/Program Manager, the Service Coordinator prioritizes a person-centric model of service delivery.

Responsibilities

  • Serves as a liaison to community agencies and seeks out new services available to eligible individuals.
  • Identifies low-cost service providers and negotiates discounts.
  • Maintains documentation of participant's use of services from community agencies.
  • Manages the provision of supportive services where appropriate.
  • Develops and maintains a Resource Directory of state and local service providers.
  • Organizes educational events and promotes participant engagement.
  • Engages volunteers within the community and/or property.
  • Assists participants in building informal support networks.
  • Conducts telephonic and/or in-person participant assessments and screenings.
  • Collaborates with care and service colleagues to achieve participant goals.
  • Provides quality customer service to all eligible individuals.
  • Affords participants access to desired community services.
  • Adheres to a follow-up and monitoring schedule.
  • Completes timely electronic and written documentation within required systems.
  • Completes Care Plans and monitors plans with participants.
  • Submits documentation needed for funders and quality performance reports.

Requirements

  • High School Diploma or GED required.
  • A bachelor’s degree in social work or a related field preferred.
  • Two or more years of experience in a social service delivery with elderly, people with disabilities, and/or family population.
  • Demonstrated working knowledge of supportive services and other resources in the area served by the project.
  • Demonstrated ability to advocate, organize, problem-solve, and provide results for the participants served.

Qualifications

  • Good communication, comprehension, and interpersonal skills.
  • Working knowledge of Windows, Microsoft Office Suite, and World Wide Web.
  • Physical Activities and Requirements: Occasional lifting up to 26-50 lbs., standing, walking, stooping, pushing, pulling, occasional climbing, occasional driving.
  • Mental Activities and Requirements: Reasoning ability, mathematics ability, language ability, and acceptable working conditions.

Skills

  • Good communication, comprehension, and interpersonal skills.
  • Working knowledge of Windows, Microsoft Office Suite, and World Wide Web.

Pay

The salary range for this position is $X - $Y per hour, depending on experience and qualifications.

Schedule

The Service Coordinator I position is typically full-time, with scheduled hours ranging from 40 to 80 per week.
