Join a compassionate, patient-focused team providing remote Medication Therapy Management (MTM) and clinical case management services to Medicaid patients across Missouri. Youll build ongoing relationships with patients, help them better understand and manage their medications, and collaborate with providers and community resources to improve health outcomes. What Youll Do Provide MTM and comprehensive medication reviews via telehealth and phoneEducate patients on medications, adherence, side effects, and chronic condition managementDevelop and monitor individualized medication treatment plansIdentify and help resolve medication-related issuesCoordinate with physicians, pharmacies, caregivers, case managers, and community health workersAssess patient needs using clinical indicators and basic physical and mental health screening toolsConnect patients with healthcare and community resources when neededMaintain accurate, timely clinical documentation and continuity of careBuild lasting relationships with patients who may have limited access to traditional healthcare What Were Looking For Required Active Missouri pharmacist licenseStrong communication and patient education skillsCompassionate, personable approach and the ability to build trust remotelyStrong organization, attention to detail, and clinical documentation skillsAbility to work independently in a remote environmentMTM, ambulatory care, Medicaid, insurance, or payer experienceExperience writing SOAP notes or similar clinical documentationExperience supporting patients with chronic conditions and complex medication regimens Why Youll Love This Opportunity ???? 100% remote???? Job Type & Location This is a Contract position based out of Saint Louis, MO. Pay And Benefits The pay range for this position is $50.00 - $55.00/hr. Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
