Remote Nurse Practitioner, Palliative Care at Home - Mount Sinai Home Health - Full Time - Day

About the role

Palliative Care at Home, part of Mount Sinai Home Health, provides home-based palliative symptom assessment, treatment, and ongoing goals of care discussions for patients and families facing advanced illness. The full clinical team includes Community Health Workers (CHWs), Nurse Practitioners (NPs), nurses, social workers, and physicians. The team collaborates with patients' healthcare teams to deliver high-quality, evidence-based, complex palliative care and chronic care management, aiming to improve symptoms, quality of life (QOL), reduce caregiver burden, decrease unnecessary inpatient utilization, and facilitate hospice when appropriate.

Responsibilities

• Conduct in-home and telehealth assessments for patients with complex needs, formulating diagnoses, ordering and interpreting diagnostics, and delivering appropriate care aligned with patient preferences and goals of care.
• Prescribe new medications and perform medication reconciliation.
• Demonstrate sound clinical judgment and evidence-based practice in symptom management and end-of-life care.
• Facilitate family meetings, goals of care conversations, and advance care planning (ACP) discussions.
• Lead oversight of care plans in conjunction with the rest of the clinical care team.
• Advocate for the patient and caregiver in collaboration with other care providers to ensure coordinated and consistent care.
• Work closely with interdisciplinary team to address changes in patient needs or symptoms.
• Educate patients and families on the trajectory of illness, use of medications, and assess disease understanding.
• Participate in weekly interdisciplinary team (IDT) meetings with the care team.
• Document as required.
• Perform other related duties as assigned.

Qualifications

• Master's degree program designed to prepare a NP to diagnose, treat, and prescribe for a patient's condition within their specialty area of practice or successful completion of an approved supplemental education program.
• Licensure: New York State License with current registration as a registered nurse.
• State Certification: Certified by the New York State Department of Education as a Nurse Practitioner with current registration as follows:
• a. Is a graduate of a New York State registered Nurse Practitioner Program or equivalent.
• b. Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course).
• c. Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner.
• d. Has a Collaborative Relationship with a physician based on employment in clinical practice in the NP role at Mount Sinai Hospital. If there is a documented work history as a Nurse Practitioner with greater than 3600 practice hours.
• Previous progressive experience relevant to the area of clinical practice required.
• For all NPs hired effective June 1, 2019, National Board Certification in Advanced Practice Nursing required. For NPs who bill for professional services, National Board Certification is required regardless of hire date. The Board Certification requirement may be deferred up to 1 year if approved by Chief Nursing Officer or designee.
• Credentialed through the Credentialing Committee of the Medical Board with approved Delineation of Privileges.
• Drug Enforcement Administration (DEA) certificate and Basic Life Support (BLS) certification required.
• Advanced Cardiac Life Support (ACLS) - ACLS recommended/preferred.
• National certification in a primary field (eg Adult, Gerontology, Family, Acute Adult, etc).
• 3+ years of palliative care/geriatrics experience.
• Experience working in interdisciplinary care teams.
• Preferred: Hospice and Palliative Certification, Home care, hospice or palliative care experience, Advanced Care Planning experience, Spanish speaking proficiency/fluency.

Compensation

The salary range for the role is $123,576 - $197,400 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need.
