Remote Nurse Care Manager (Compact License – Multi-State)

Key Responsibilities

• Deliver comprehensive care management services across diverse patient populations
• Conduct patient assessments and develop individualized care plans
• Support chronic disease management (e.g., diabetes, hypertension, CHF, COPD)
• Provide patient education on conditions, medications, and self-management strategies
• Use motivational interviewing techniques to improve adherence and engagement
• Care coordination with providers, pharmacists, and other healthcare professionals
• Identify care gaps, risks, and barriers to treatment adherence
• Ensure accurate and timely documentation in compliance with program standards
• Support care transitions and continuity across settings

Required Qualifications

• Active Registered Nurse (RN) Compact License (NLC)
• Minimum of 2 years’ experience in care management, case management, or chronic disease management
• Willingness to work across multiple state populations

Preferred Qualifications

• Experience in: Chronic care management programs (CCM), Transitional care management (TCM), Remote patient monitoring (RPM), Telehealth or virtual care delivery
• Strong background in patient education and engagement
