Key Responsibilities
- Deliver comprehensive care management services across diverse patient populations
- Conduct patient assessments and develop individualized care plans
- Support chronic disease management (e.g., diabetes, hypertension, CHF, COPD)
- Provide patient education on conditions, medications, and self-management strategies
- Use motivational interviewing techniques to improve adherence and engagement
- Care coordination with providers, pharmacists, and other healthcare professionals
- Identify care gaps, risks, and barriers to treatment adherence
- Ensure accurate and timely documentation in compliance with program standards
- Support care transitions and continuity across settings
Required Qualifications
- Active Registered Nurse (RN) Compact License (NLC)
- Minimum of 2 years’ experience in care management, case management, or chronic disease management
- Willingness to work across multiple state populations
Preferred Qualifications
- Experience in: Chronic care management programs (CCM), Transitional care management (TCM), Remote patient monitoring (RPM), Telehealth or virtual care delivery
- Strong background in patient education and engagement
