Remote/Hybrid Nurse Practitioner, Primary Care

The Role

This is a primarily telehealth-based position; most of your work will be done virtually, with the occasional option to see patients in person if they're local to you. You'll be part of building something new: an innovative virtual and in-person care model focused on providing primary care for patients with CKD and related chronic conditions.

Monday–Friday only
Self-managed schedule
No nights, no weekends, no on-call

Requirements

• Minimum 5 years of NP experience (primary care, internal medicine, cardiology, nephrology, or case management)
• Experience managing chronic diseases (CKD, CHF, diabetes, hypertension)
• Active, unrestricted NP license in Washington State
• DEA license (or ability to obtain within 90 days)
• Current CPR/BLS certification
• Comfortable working across multiple EMR systems
• Strong computer skills — MS Word, Excel, Outlook

PREFERRED QUALIFICATIONS

• Telehealth experience (strong plus)
• Value-Based Care or Population Health background
• Medicare Advantage risk documentation/coding experience
• Experience reducing avoidable hospitalizations through proactive, coordinated care
• Familiarity with palliative care discussions
