Join our Shields team as an Ambulatory Clinical Pharmacy Specialist, playing a hands-on role in elevating patient care at a nationally recognized Academic Medical Center. This dynamic, patient-centered position allows you to build meaningful relationships with patients and providers while delivering high-quality medication management remotely.

About the Role

You’ll collaborate directly with care teams and practice under a Collaborative Practice Agreement (CPA) to conduct virtual patient assessments, order and monitor evidence-based treatment plans, optimize therapy, and resolve medication-related problems. This is a remote role supporting the Centralia/Olympia/Lacey area.

Potential on-site locations include Providence Centralia Hospital and Providence St. Peter Hospital.

Responsibilities

  • Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug-level assessments aligned with evidence-based standards.
  • Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA and promote seamless medication coordination.
  • Monitor therapy effectiveness and safety, identify optimization opportunities, and intervene as appropriate.
  • Support quality-improvement and REMS initiatives.
  • Provide recommendations to operations by supporting teammates with complex prior authorizations, improving workflows, educating patients, and maintaining ongoing clinical development.

Requirements

  • Hold a Doctor of Pharmacy (PharmD).
  • Must be a licensed pharmacist in Washington.
  • Meet one of the following qualifications:
    • Completion of a PGY2 Ambulatory Care Residency.
    • Completion of a PGY1 Residency and at least 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA).
    • At least 3 years of ambulatory practice with disease state management.
  • Strong communication, organization, and technical skills.
  • Empathy, integrity, and a team-first mindset.
  • Proficiency in Microsoft Office tools (Outlook, Teams, Word, Excel, and PowerPoint) for communication, documentation, and data management, as well as navigating the internet and web-based platforms.

Data entry and computer skills will be evaluated through an assessment as part of the pre-employment process.

Pay

Salary range: $127,000 – $142,000 per year (Hourly Equivalent: Approximately $61.06 - $68.27 per hour, based on a 40-hour workweek). The actual salary will depend on experience, geographic location, and other factors permitted by law. Eligibility for bonus pursuant to applicable bonus plan terms.
