Remote Clinical Pharmacist – New Jersey

About the role

PharmD Live is seeking an experienced Clinical Pharmacist licensed in New Jersey to join their growing virtual care team. This role focuses on delivering pharmacist-led clinical services that help patients better manage chronic conditions, improve medication adherence, and achieve positive health outcomes.

Primary Responsibilities

• Conduct comprehensive medication reviews and medication reconciliation services
• Identify, assess, and resolve medication-related problems
• Provide telephonic and virtual patient consultations
• Support patients with chronic conditions including diabetes, hypertension, heart failure, COPD, and other complex disease states
• Develop individualized medication management recommendations for providers and patients
• Promote medication adherence through patient coaching and motivational interviewing
• Participate in care transition and post-discharge medication review activities
• Document clinical encounters accurately and in accordance with regulatory and organizational standards
• Collaborate with physicians, nurse practitioners, and care teams to optimize patient outcomes
• Contribute to quality improvement initiatives and patient engagement efforts

Minimum Qualifications

• Doctor of Pharmacy (PharmD) degree from an accredited institution
• Active New Jersey Pharmacist License in good standing
• Minimum of two (2) years of pharmacist experience in ambulatory care, clinical pharmacy, managed care, telehealth, or related settings
• Strong computer proficiency and ability to navigate multiple clinical systems
• Ability to work independently in a remote environment while maintaining productivity and quality standards
• Willingness to obtain additional pharmacist licenses as business needs evolve
• Ability to educate patients in Spanish language

• Board Certification (BCPS, BCGP, CDCES, or similar)
• Meditation Therapy Management (MTM)
• Chronic Care Management (CCM)
• Transitions of Care (TCM)
• Remote Patient Monitoring (RPM)
• Population health or value-based care programs
• Patient engagement and health coaching
