Remote Clinical Pharmacist – HEDIS Medication Adherence

About the Role

We are seeking a highly motivated and detail-oriented Clinical Pharmacist with a PharmD degree and experience in HEDIS medication adherence initiatives. The ideal candidate will play a crucial role in improving medication adherence rates among patients, ensuring compliance with HEDIS/Medicare quality measures, and driving overall health outcomes.

Responsibilities

• HEDIS Quality Measures: Lead efforts to improve HEDIS medication adherence and other pharmacy quality measures, including but not limited to adherence to antihypertensive, cholesterol-lowering, and diabetes medications. Analyze medication adherence data, identify gaps in care, and implement targeted interventions to improve adherence rates.
