Remote Clinical Pharmacist

This role supports clinical priorities focused on quality, patient-centered outcomes, and effective care delivery. You will collaborate with clinical leaders, operational teams, and cross-functional stakeholders to improve standards, workflows, and execution within a healthcare environment.

Responsibilities

• Complete thorough medication reconciliation for patients
• Identify and address medication-related issues during consultations
• Communicate clinical recommendations effectively to healthcare providers
• Provide both initial and follow-up medication counseling to patients
• Maintain patient scheduling and coordinate follow-up assessments
• Act as a financial advocate by exploring affordability options for medications
• Manage prior authorizations and handle any denials or appeals

Requirements

• PharmD degree from an accredited institution required
• Unrestricted pharmacist license in Pennsylvania required
• 1-3 years of experience in retail/ambulatory care pharmacy preferred
• Experience with clinical pharmacy practices and medication management
• Knowledge of 340B purchasing program is beneficial

Location: Remote - US based candidates only, no visa sponsorship available

Pay

$110,000 – $130,000 annually
