About the role

Continuum is adding remote, nationwide positions for Social Work Care Navigators responsible for resource referral, counseling, and coordination of care transitions for older adults with dementia and their caregivers.

Hours are flexible to your schedule.

Responsibilities

• Communicate well with clients, families, healthcare providers, and resources
• Coordinate transitions of care
• Identify resources that could enhance independence and client/caregiver support

Requirements

• Hold a BSW or MSW degree or equivalent
• Have experience working with older adults and memory impairment
• Pass a background check
• Have a positive, engaging, proactive approach

Qualifications

• Understand dementia and its challenges for clients and caregivers
• Gain rapport easily with others
• Enjoy working with older adults and their families
• Effectively manage your time to meet the position’s needs
