Remote Call Post-Acute Nurse Practitioner - 62017BR

Key Responsibilities

• Triage, implement, and adjust individualized care plans based on patient needs
• Collaborate with nursing staff, physicians, and other healthcare professionals in a team approach
• Maintain accurate, timely documentation in compliance with regulations

Qualifications

• Multiple state NP licenses are required; candidates with active New York and Maryland NP licenses are strongly preferred
• Experience in post-acute and/or telephone triage required
• Strong clinical foundation and knowledge of healthcare regulatory standards
• Excellent communication and interpersonal skills
• Collaborative, team-oriented approach with a positive outlook
• Weekend availability required

Pay

Estimated range of $60 - $60 hourly

Schedule

Full-time and part-time opportunities available, providing weekend clinical telephonic call coverage for skilled nursing facilities.
