Rehabilitative Service Aid
Description
Envision Unlimited is a full-spectrum care organization serving people with disabilities from infants to 80+ years old, regardless of race, gender, religion, or ability to pay. Founded in Chicago in 1948, Envision Unlimited provides innovative services across Chicagoland and Central Illinois, including community living options, novel day programs, behavioral health services, employment programs, respite for families, and foster care for children with disabilities.
Purpose
The Rehabilitative Service Aid (RSA) will work directly with clients who have transitioned from nursing homes and long-term care facilities into their own apartments within the community. RSA will participate as a member of the multidisciplinary team responsible for the supervision, training, and implementation of an individual's treatment plan and behavior management plans. RSA will provide direct service to all members and complete documentation as required by the multidisciplinary team, CARF Accreditation, and funders including Medicaid standards.
Essential Responsibilities
- Meet established productivity targets as assigned.
- Must provide program supervisor with a mobile number in which you can be reached and be accessible via mobile phone during work hours.
- Provide compassionate care and recovery-oriented services to members with mental illness and/or other disabilities.
- Aid members to achieve independent living goals by providing training in key areas such as: activities of daily living, budgeting, transportation, developing healthy relationships, and other items defined by treatment plan goals.
- Assist in the development and adherence to a healthy menu (based on client’s dietary needs) and in the preparation of meals as indicated by the client’s skill level according to their service and/or treatment plan.
- Provide and supervise recreational activities with members both on site and in the community.
- Supervise members in the self-administration of medications.
- Provide training/education to them on the process, usage, and side effects of their medications.
- Follow-up with supervisor and/or medical professionals around any issues or concerns that happen related to medications.
- Document services to members as outlined in their treatment plan in individual or group note forms as applicable. Notes must be compliant with agency standards and completed before the end of each shift.
- Assure safe, clean, and secure living environment for all members by assisting them to complete household chores.
- Complete cleaning and other tasks as necessary by the end of the shift.
- Attend all supervisions and trainings as required and/or instructed by the Program Supervisor or Director of Mental Health.
Overall Job Performance
- Adhere to all the Policies and Procedures of Envision Unlimited.
- Maintain confidentiality according to the Illinois Mental Health code and Confidentiality Act.

Requirements
- Must be at least 21 years of age or older.
- Must be able to pass a required criminal background check and TABE test.
- Must have a Valid IL Driver’s License and a High school diploma or GED equivalent.
- Must have excellent writing and communication skills, 1 year of social service experience preferred.
- Must be flexible to work at the 1801 S. Wabash site or the 1533 W. Warren site.
Pay
$20.00 per hour
