Rehabilitation Specialist - Transitional Age Youth

About the role

Provide support and services to young adults aged 18-23 in a residential and psychiatric rehabilitation program focused on fostering independence and community integration.

Responsibilities

• Build relationships with youth and develop personalized plans.
• Promote self-sufficiency and involve families and community partners.
• Coordinate assessments and facilitate access to services.
• Document progress and implement quality improvement measures.
• Engage youth through meaningful activities.
• Assist with orientation and engagement strategies.

Requirements

• High school diploma or GED required; bachelor’s degree preferred.
• Two years of relevant experience working with transitional age youth in social or educational settings.
• Proficiency in Microsoft Office.
• Valid driver’s license with minimal points and access to a vehicle.
• Willingness to obtain additional certifications.
• Ability to work flexible hours, including weekends and holidays.
