Rehabilitation Specialist - Evening Shift

About the Role

Assist individuals in their recovery process by supporting daily living activities, monitoring medication adherence, facilitating transportation, and providing community-based behavioral health support to promote independence.

Responsibilities

• Prompt residents and support daily living activities to aid in recovery
• Monitor medication adherence
• Facilitate transportation as needed
• Provide community-based behavioral health support to promote independence

Requirements

• High School Diploma or GED
• Experience in mental health or psychosocial rehabilitation
• Valid driver's license
