About the role

Rehab RN responsible for providing resident-centered rehabilitation nursing care to seniors in a faith-informed community.

Responsibilities

• Assess residents and develop individualized care and rehab plans.
• Administer medications and treatments according to care plans.
• Collaborate with therapists, physicians, CNAs, and families to support recovery, independence, and quality of life.
• Document care accurately in the EMR system.
• Uphold safety and regulatory standards.
• Contribute to a team-oriented environment focused on dignity, respect, and holistic well-being.
