Rehabilitation Counselor - PACT - Relief
Rehabilitation Counselor - PACT Riverside Community Care is seeking a Rehabilitation Support Counselor member to join our community-based Program for Assertive Community Treatment (PACT) program based in Needham and serving the surrounding area. PACT provides individuals referred by the Department of Mental Health or MBHP with access to community-based rehabilitation, clinical treatment, employment, housing, wellness, and peer services, with ancillary supports coordinated in partnership with the individual. Services are designed to be flexible based on the needs of the persons served and to promote resiliency and recovery for all participants. PACT is a 24/7 program and services are provided 365 days per year.
About the Role
Working as part of a multi-disciplinary team, staff will provide guidance, education, supervision, and direct supports to allow individuals served to maintain their physical and emotional health, live safely in the community, and develop the skills necessary to meet their goals. Interactions will be guided by an appreciation for those being served as unique individuals and adults, with the expectation and goal of mutual learning and growth. Services will be provided in keeping with the Division Mission Statement and PACT Guidelines and are consistent with Riverside and Department of Mental Health standards and requirements.
Responsibilities
- Provide guidance, education, supervision, and direct supports to individuals served
- Support individuals in maintaining physical and emotional health
- Assist individuals in living safely within the community
- Help individuals develop skills to meet their personal goals
- Provide transportation to persons served in a personal vehicle, as needed
Requirements
- Bachelor’s Degree and one year of experience or High School Diploma and a minimum of three years related experience
- Experience working with adults who have severe mental illness
- Criminal Justice or Vocational Rehabilitation background preferred
- Valid driver's license and available, dependable transportation
- Willing to provide transportation to persons served in a personal vehicle, as needed

- Excellent organizational skills
- Excellent written and verbal communication skills
- Basic computer fluency (including Microsoft Office)
Pay
$19.56/hourly (relief hours available)
