About the Role

We are seeking a dedicated Rehabilitation Case Manager to coordinate and oversee comprehensive client treatment plans from admission through discharge. In this role, you will act as a primary client advocate, bridging the gap between clinical teams, families, physicians, and funding sources to ensure cohesive, high-quality care and continuous progress toward rehabilitation goals.

Responsibilities

  • Case Management & Advocacy: Manage assigned caseloads, lead interdisciplinary treatment team meetings, and advocate for client needs across all phases of care.
  • Treatment Planning: Monitor, evaluate, and revise individualized client rehabilitation goals in close collaboration with the Clinical Director and primary healthcare providers.
  • Stakeholder Communication: Maintain regular, transparent communication with clients, families, and funders regarding progress, schedule updates, and care expectations.
  • Financial & Funder Coordination: Monitor funding guidelines, facilitate alternative or future funding sources, and ensure accurate, timely documentation for billing integrity.
  • Clinical Documentation: Maintain accurate client charts, oversee staff log notes, and comply with state regulatory standards and professional guidelines.
  • Direct Care & Support: Provide occasional direct care support or transportation to medical appointments as needed; participate in the program's on-call rotation.

Requirements

  • Education: Bachelor's degree in a relevant healthcare or human services field required (Master's degree preferred).
  • Certifications: Certified Rehabilitation Counselor (CRC) or Certified Case Manager (CCM) preferred.
  • Experience: Proven experience in case management within the healthcare industry, with strong skills in crisis management, treatment plan formulation, and needs assessment.
  • Licensing: Valid driver's license and a clean driving record required.
  • Skills: Outstanding writing, organizational, time management, and inter-personal communication skills.
  • Mobility & Lifting: Ability to stand, walk, sit, bend, and occasionally lift/move 50+ lbs.
  • Travel: Up to 30% travel to support client appointments or visit program locations.
