About the Role

Assess, plan, and coordinate rehabilitative care for adults with complex medical and functional needs. Collaborate closely with physicians, therapists, and an interdisciplinary team to deliver holistic, patient-centered care.

Responsibilities

• Administer medications and provide care coordination.
• Educate patients and families.
• Conduct discharge planning.
• Maintain accurate documentation.

Requirements

• Experience in rehab or med-surg nursing.
