Responsibilities

• Implement innovative solutions to improve patient outcomes
• Provide high-quality care in various settings including homes and long-term care facilities
• Collaborate with interdisciplinary teams to develop comprehensive care plans
• Ensure compliance with regulatory standards and best practices
• Communicate effectively with patients, families, and other healthcare providers

Requirements

• High school diploma or equivalent
• Valid driver’s license and reliable transportation (if applicable)
• Strong interpersonal skills and ability to build rapport with patients and families
• Ability to work flexible hours, including evenings and weekends
• Passing score on a background check

Qualifications

• Bachelor's degree in a relevant field (physical therapy, nursing, etc.) preferred
• Experience in healthcare or related field
• Knowledge of healthcare regulations and standards

• Ability to work independently and as part of a team
• Attention to detail and organizational skills
