Responsibilities
- Implement innovative solutions to improve patient outcomes
- Provide high-quality care in various settings including homes and long-term care facilities
- Collaborate with interdisciplinary teams to develop comprehensive care plans
- Ensure compliance with regulatory standards and best practices
- Communicate effectively with patients, families, and other healthcare providers
Requirements
- High school diploma or equivalent
- Valid driver’s license and reliable transportation (if applicable)
- Strong interpersonal skills and ability to build rapport with patients and families
- Ability to work flexible hours, including evenings and weekends
- Passing score on a background check
Qualifications
- Bachelor's degree in a relevant field (physical therapy, nursing, etc.) preferred
- Experience in healthcare or related field
- Knowledge of healthcare regulations and standards
