Rehab Liaison - Inpatient Rehab

Liaison Duties

• Work with all appropriate members of the health care team to provide coordination of admissions and care through the continuum of services.
• Maintain compliance with time requirements for completion of Preadmission Screens.
• Achieve and evaluate specific patient outcomes, addressing variances as necessary.
• Utilize resources and services appropriately by reviewing admissions, duration of stay, and professional services rendered.
• Consult with system staff (Administration, LSW, RN, MD) for complicated issues.
• Market services to assigned territory, meeting minimum monthly referral quotas as indicated by Program Director.
• Engage in networking and travel up to 50% of time, including unit tours, community outreach events, and physician networking.

Requirements

• Education: Bachelor's Degree or equivalent.
• Licensures/Certifications: Current Clinical license in the state of SC (RN, PT, OT, SPL, RT) and BLS.
