Rehab Community Services Coordinator

Organize and oversee the day-to-day operations and program development of the supervised exercise program, collaborating with the community aquatics program. Ensure continuous supervision and routine provision of services for participants, assist with patient recertification, perform clerical duties, transport patients, coordinate infection control, maintain and order supplies, and manage volunteers for the rehab departments.

Responsibilities

  • Ensure consistent quality of care for all program participants, addressing race, religion, cultural, and financial needs.
  • Engage in non-patient care activities for hospital quality improvement, professional growth, and alignment with the hospital’s mission.
  • Meet skills competency requirements and document observations in behavioral terms.
  • Translate theory and philosophy into practical formats and establish rapport with patients, families, physicians, students, volunteers, staff, and the public.
  • Oversee operations of the Supervised Exercise Program, including scheduling participants, collecting fees, and preparing financial reports for the Manager of Rehab Therapy Services.
  • Complete charge reconciliation and reports for OT, PT, and ST services as instructed.
  • Track and report staff productivity.
  • Teach group exercise classes and provide one-on-one exercise instruction.
  • Order supplies and equipment for OT, PT, ST, and EMG clinic as requested.
  • Operate the Rehab Van to transport patients and staff for home visits or community outings.
  • Accurately take and explain blood pressure and pulse oximetry readings to community participants.
  • Generate reports using spreadsheets and organizational skills as requested.

Requirements

  • American Heart Association Basic Life Support (BLS) Certification.
  • Current CPR certification.
  • Knowledge of growth and development, the impact of hospitalization on patients, and group dynamics.
  • Ability to pass water safety competencies and serve as a second person or instructor in the aquatics program.
  • Competency in the safe operation and use of all gym and pool equipment.
  • Competency in emergency pool safety.
  • Valid driver’s license and clean driving record to operate the Rehab Van.
  • Degree in Sports Management, Recreation and Leisure Studies, Exercise Physiology, or Athletic Training (certified or eligible), or related field. One year of aquatic programming experience may substitute for one year of formal education (up to two years).

Qualifications

  • One year of experience as an exercise specialist preferred.
  • Certification by the Arthritis Foundation of North Carolina to teach pool classes preferred.
  • Strong communication skills and ability to interact positively with participants and coworkers of varied backgrounds.
  • Knowledge of spreadsheets and organizational skills for report generation.

Pay

$19.11 - $27.86 per hour.
