Registered Practical Nurse (RPN) - Casual - Ridgeview

About the role

Reporting to the Director of Care (DOC) or Director of Health and Wellness (DHW), the Registered Practical Nurse (RPN) provides planned, patient-oriented, goal-directed nursing care in accordance with provincial standards of care.

Responsibilities

• Work as a member of the inter-professional team applying the person-centered approach to care.
• Complete and ensure residents' clinical care assessments, care/service plans, and documentation are accurate, up-to-date, and changes communicated as required.
• Implement and evaluate the effectiveness of care/service plans and make changes as needed.
• Assist with scheduling residents' care conferences and participate where applicable.
• Work closely with DOC/DHW to facilitate residents' move-in, move-out, and transfers.
• Provide direct resident care, medication/treatment administration, and document care in compliance with regulatory and Extendicare Resident Care standards.
• Process orders, safely manage drugs and narcotics, and administer them in compliance with regulatory requirements, scope of practice, and Extendicare policies.
• Facilitate and provide resident/family education.
• Plan, coordinate, manage, and evaluate the resident's care plan in collaboration with the resident/substitute decision-maker, physicians, nursing, and interdisciplinary team.
• Coordinate nursing team's resident care assignments to ensure efficient and timely delivery of services.
• Respond to and manage significant changes in residents' conditions and medical emergencies; provide emergency first aid.
• Teach, delegate to, and direct unregulated care providers.
• Address resident/family concerns, ensuring appropriate action is taken and concerns are communicated to DOC/DHW/Executive Director and other departments.
• Assist the DOC/DHW in coordinating, implementing, and monitoring Infection Prevention and Control, Health and Safety, and Clinical Programs and Quality/Safety Plans for the Nursing Department.
• Uphold and promote the organization's values and philosophy relating to ethics, morality, and integrity as set out in Extendicare's Code of Conduct.
• Complete all other duties as assigned.

Qualifications

• Current registration as a Registered Practical Nurse in the applicable Province/State.
• Experience working in a seniors' environment and with unregulated care providers preferred.
• Education and experience in gerontology, rehabilitation, community/complex/long-term care are assets.
• Strong written and oral communication, critical thinking, care coordination, time management, and customer focus skills.
• Demonstrated sound clinical knowledge.
• Current Basic Cardiac Life Support and First Aid certification.
• Computer proficiency and experience with Microsoft Office applications.
• Current (within 6 months) Vulnerable Position/Sector Screening (VPS).

Pay

$33.71 - $35.24 per hour. Final salary offers are based on skills, experience, education, and alignment with the responsibilities of this position.
