Responsibilities

• Provide one-on-one, patient-centered care in a home setting
• Perform skilled assessments, administer treatments, and educate patients and families on disease management and medications
• Work independently while collaborating with a supportive multidisciplinary team
• Build meaningful relationships and become a trusted advocate for your patients

Education/Experience Requirements

Required:

• A graduate of an accredited institution of professional nursing
• One year experience in a medical surgical or related setting

Preferred:

• An associates or Bachelors of Science in Nursing from an accredited nursing program
• Experience in Home Care

Licensure/Certification Requirements

Required:

• Current New York State Licensure as a Registered Nurse
• Current and valid NY State Driver's License in good standing

Preferred:

• Basic Life Support (BLS) Certification - must receive BLS certification within six (6) weeks of hire date
