Visiting Health Services of New Jersey (VHSNJ), a partnership between Hackensack Meridian Health and St. Joseph's Health, provides comprehensive home health care and hospice services in Passaic, Bergen, and parts of Morris counties. Our multidisciplinary teams deliver high-quality care where patients feel most comfortable.

This role offers a flexible schedule with fewer distractions, allowing you to spend meaningful one-on-one time with patients.

Responsibilities

• Provide professional nursing care to patients in their homes through assessment, development, implementation, and evaluation of home nursing care needs.
• Ensure compliance with physician orders, including timely visits and documentation reflecting progression toward discharge goals.
• Demonstrate holistic patient care, ensuring safe and effective outcomes through clinical judgments and critical thinking.
• Complete assigned treatments and procedures according to established standards and policies.
• Utilize the interdisciplinary team and document all activities accurately.
• Identify appropriate payors and comply with authorization requirements for self and other disciplines.
• Maintain visit frequency within authorized ranges and use secure messaging for insurance communications.
• Complete nursing assessments, medication reconciliation, and collaborate with physicians on the Plan of Care.
• Update care plans within two weeks of changes in patient status and implement teaching strategies aligned with care goals.
• Report patient care problems to management or physicians promptly and document case communications.
• Act as a liaison among physicians, managers, and healthcare team members to coordinate services and resources.
• Communicate patient information and Plan of Care during transfers or scheduled time-off.
• Complete routine assignments independently, accurately, and consistently in a timely manner.
• Meet productivity requirements consistently.
• Act as a clinical supervisor for para-professional home health aide staff, completing supervisory visits within regulatory time frames.
• Participate in the evaluation of home health aide staff.
• Maintain awareness of updated standards and incorporate them into nursing practice.
• Demonstrate flexibility with job responsibilities and adhere to organizational competencies and standards of behavior.
• Perform physical tasks such as lifting a minimum of 50 lbs., pushing/pulling 50 lbs., and standing for at least 8 hours a day.
• Perform other duties and projects as assigned.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing.
• Minimum of six (6) months of RN experience or at least one year of LPN experience, preferably in home care or hospice.
• PC experience and skills.

Skills

• NJ State Professional Registered Nurse License.
• AHA Basic Health Care Life Support HCP Certification.
• IV Certification (preferred).

Pay

The starting rate of pay is a reasonable good faith estimate of the minimum base pay for this role in accordance with the New Jersey Pay Transparency Act. The posted rate may be adjusted based on factors such as labor market data, experience, education, certifications, skills, geographic location, internal equity, and budget considerations.

Minimum annual rate: $94,161.60.
