About the Role

The RN Triage Nurse provides professional nursing care for practice patients following established standards and practices. Acts as the primary liaison between patients, staff, and physicians, coordinating patient care and responding to patient inquiries with updates and education.

Responsibilities

  • Speaks directly with patients to assess needs, determine urgency, and manage calls to an appropriate conclusion, responding to urgent calls within ½ hour and non-urgent calls in a timely manner.
  • Performs wound care, dressing changes, suture and staple removal, and casting as required (Nurse Visits).
  • Provides basic patient care as ordered and documents all activities, interventions, patient/family responses, and medication dispensed/prescribed in the electronic medical record (EMR).
  • Promotes wellness by providing patient education materials and communicating physician advice/instructions.
  • Adheres to infection control/safety guidelines and confidentiality policies.
  • Dispenses durable medical equipment (DME) as requested by the provider and documents supplies in the EMR.
  • Assists with prescription preparation, entry into the patient’s chart, and electronic transmission to pharmacies, ensuring 100% documentation in the patient’s chart.
  • Documents prescriptions for other treatment modalities such as PT, OT, and Aqua Therapy in the patient’s chart.
  • Follows up on lab work and diagnostic tests, including scheduling urgent diagnostic testing.
  • Maintains patient confidentiality.

Requirements

  • Current unrestricted RN license.
  • Minimum of 2-3 years of nursing experience in an acute care or hospital setting or case management experience.
  • Preferred experience with discharge planning, outpatient care coordination, and orthopedic care.
  • Excellent interpersonal and customer service skills.
  • Strong problem-solving skills with a flexible and creative approach.
  • Ability to work independently and multitask in a fast-paced setting.
  • Excellent oral and written communication skills, including the ability to communicate professionally with healthcare teams and clearly with patients and families.
  • Appropriate telephone etiquette for telephonic duties.

  • Clear, concise, and organized verbal and written communication.
  • Analytical skills to break down problems and develop logical solutions.
  • Ability to exercise good judgment within company policies and procedures.

Physical Demands

  • Must be able to lift up to 30 pounds.
